Shift Details
- Schedule: Day Shift (4 x 10 hours)
- Hours: 7:00 AM to 5:00 PM
Requirements
- Minimum of 1 year of pharmacy experience
- Valid Oregon Pharmacist License (required)
- Chemo/Hazardous Drug certification (required)
- Experience with Home Health Infusion settings (required)
- Familiarity with inpatient/acute care environments
- Proficiency with Pyxis medication dispensing systems (required)
- Preferred experience with Epic charting system
- Preferred community hospital experience
- Travel experience is not required for this role
Key Responsibilities
- Dispense medications accurately using Pyxis systems
- Provide pharmaceutical care to patients in home health infusion and inpatient/acute care settings
- Ensure safe handling and administration of chemotherapy and hazardous drugs following established protocols
- Collaborate with healthcare teams to optimize medication therapies and improve patient outcomes
- Maintain comprehensive and accurate documentation in preferred charting systems such as Epic
- Educate patients and caregivers on medication usage, potential side effects, and storage requirements
- Adhere to all regulatory and safety standards governing pharmacy practice
Qualifications
- Licensed Pharmacist in the state of Oregon
- Certification in chemotherapy and hazardous drug handling
- Strong knowledge of medication dispensing technologies, with proficiency in Pyxis
- Excellent communication and interpersonal skills
- Detail-oriented with strong organizational capabilities
- Ability to work independently and collaboratively within a multidisciplinary team
